Your First Visit
Understanding Our Fees and Paperwork
Your understanding of the therapeutic process is important to us. Our understanding of the purpose of your visit is equally important. All of the paperwork you will need is available below. You have the option of downloading the paperwork and filling it out before you come or we will have the forms sitting out for you in the waiting area. The forms take about 15 minutes to fill out, so please arrive early if you plan to fill them out in the office.
Your personal information is protected by state and federal laws. On your first visit your therapist will review the forms with you to be sure that you understand your rights and responsibilities as a client of Bozeman Center for the Healing Arts, Inc., as well as your therapist's responsibilities to you. The client intake form will help your counselor become aware of the personal goals and issues that have led you to seek therapeutic consultation.

Fees & Insurance
The fees of our licensed therapists, depend on their credentials and years of experience. We request payment at the beginning of each session. Please ask your insurance company if Bozeman Center for the Healing Arts, Inc. may be covered. If you will be covered, you must pay upfront and be reimbursed by your insurance company. If not and you wish to use your out-of-network benefits, we ask that you have your insurance company reimburse you.
Payments of cash, check, credit or debit cards are accepted.
We uphold a strict 24-hour cancellation notice. If you fail to notify your therapist of a cancellation, you agree to pay the full appointment fee.
